How is atrial fibrillation diagnosed?
Dr. Curtis, how is atrial fibrillation diagnosed?
On physical examinations, sometimes we can get a pretty good clue
because we can detect a rapid irregular heartbeat.
But the real gold standard is any kind of the ECG or EKG monitoring.
If we get an electrocardiogram on a patient
and they are having atrial fibrillation right then, we can diagnose it very easily.
However there are some patients who will go in and out of atrial fibrillation,
they may alternate between a normal rhythm and the abnormal rhythm.
That requires longer term monitoring.
So thereâ™s some tests like Holter monitors where we put it on the patient for twenty four hours.
Again, if they've had an episode weâ™ll be able to detect it.
There a longer term monitors that we can use externally for up to about a month or so,
and again if there are infrequent episodes, weâ™ll be able to detect that.
The bottom line is it has to be an electrocardiographic (or EKG) recording of the rhythm at the time the patient has it.
Absolutely.
So, for the patient with stroke or suspected stroke, it would be important to look for atrial fibrillation as well.
Just like you were saying, the long term monitoring would be very helpful.
Right. And if we see it at the time when they come in,
then we get a pretty good idea of what the cause of the stroke was.
